What does the clinical research about creatine and HMB/D3?

Effect on Quality of Life and Handgrip Strength by Dynamometry of an Enteral Specific Supplements with Beta-Hydroxy-Beta-Methybutyrate and Vitamin D in Elderly Patients

In this 12-week study, 35 subjects with the mean age of 79.1+7.5 years were examined to see how the nutritional intervention of HMB and Vitamin D would effect strength and quality of life. The results from the study confirm the beneficial effects on weight, handgrip strength, some domains of quality of life and Vitamin status of an enhanced enteral formula with Vitamin D and beta-hydroxy beta-methylbytyrate (HMB) in elderly patients.

Effect of Calcium β-Hydroxy-β-Methylbutyrate (CaHMB), Vitamin D, and Protein Supplementation on Postoperative Immobilization in Malnourished Older Adult Patients With Hip Fracture: A Randomized Controlled Study

A clinical study examined how the combination of myHMB, Vitamin D3, and protein could help improve recovery after orthopedic surgery for a hip fracture. Patients supplementing with the myHMB + Vitamin D3 , and protein cocktail were more mobile on days 15 and 30. The patients were also found to have significantly increased strength on day 30.

Long-term Effects of Calcium beta-Hydroxy-beta-Methylbutyrate and Vitamin D3 Supplementation on Muscular Function in Older Adults With and Without Resistance Training: A Randomized, Double-blind, Controlled Study.

In this year-long study of 117 older adults (≥60 y) were studies in four treatment arms: 1) 3 g HMB plus 2,000 IU Vitamin D per day with exercise; 2) 3 g HMB plus 2,000 IU Vitamin D per day with no exercise; 3) Placebo with exercise; and 4) Placebo with no exercise.  All participants had baseline Vitamin D levels from 15-30 ng/mL, which would be considered insufficient for optimal physical functioning.  HMB plus Vitamin D supplementation significantly increased lean mass within the non-exercising group at 6 months.  Additionally, in the non-exercising groups knee extension peak torque (60⁰/s) was significantly improved at 3 months in the HMB plus Vitamin D group compared with the non-exercising placebo group.  A functional index of the physical tests, which included handgrip strength, get up, and get up and go measurements, showed that HMB plus Vitamin D resulted in a significant increase in the functional index at 3, 6, and 12 months of the study.  The authors concluded that HMB plus Vitamin D could enhance muscle strength and physical function in older adults even without engaging in an exercise program.

Vitamin D status affects strength gains in older adults supplemented with a combination of β-hydroxy-β-methylbutyrate, arginine, and lysine: a cohort study.

This year-long retrospective study in elderly adults demonstrated that a nutrient mixture of HMB, arginine, and lysine alone was effective in improving body composition regardless of vitamin D status, but accompanying strength increases were observed only when participants also had adequate vitamin D status indicating a synergistic effect between the HMB, arginine, and lysine and vitamin D.

A three-week study examined whether creatine and HMB have similar or different effects on increasing lean body mass and strength in individuals undergoing resistance exercise training. Results showed that all groups experienced gains in lean body mass, with the creatine and creatine/HMB groups showing significant increases compared to the placebo group. The findings also suggest that creatine and HMB have additive effects on lean body mass and strength, potentially acting through different mechanisms.
